Eligibility
The Health Service is available to all UW-Stevens Point students who are enrolled in the current semester. All students are required to complete a Detailed Medical History (DMH) or submit a history and physical examination from their private physician prior to being seen at the Health Service. The Detailed Medical History is free and can be completed online.
Summer Eligibility
Students enrolled in summer classes pay a fee as part of their tuition and are eligible to be seen. Students enrolled this spring and registered for fall semester may pay a pre-paid fee equal to what summer students pay in their tuition ($49.20 for 2008) to be eligible to be seen for the same services as regular enrolled summer students. Arrangements for paying this fee can be made at the University Health Service. Students wishing to sign up for this option should visit Joan Schuh, Program Assistant, during regular business hours. Her office is on the first floor of Delzell Hall.
The deadline to enroll in this pre-payment option is the Friday following finals week. Students enrolled this spring and registered for fall semester who chose not to enroll the deadline can be still be seen, space available, by using a fee-for-service model that includes $60 for the visit plus any other charges incurred.
