Assignments for the Week

  1. Take Quiz in D2L (open Monday, Nov 3 @ 12:01 a.m. until Sunday, Nov 9 @ Midnight).  The quiz will cover Chapter 2 and concepts presented in class this week.
  2. Setup your log for your Personal Behavior Change Plan due by Noon, Monday, Dec 8, 2008

    Each student will create a behavior change plan to achieve their individual wellness goal. Each student has already identified a meaningful wellness goal that can be carried out for a 3 week period (the SMART goal created last week).

    Students will carry out their plan for at least 3 weeks, analyze progress over those weeks, and plan for new efforts. Students are expected to keep information in the log form (see below). To complete an appropriate wellness behavior change plan, follow the sample behavior change plan

    Additional tips about keeping a log are below:

    1. A record or log of activities must be kept throughout the 3 weeks of effort to achieve the wellness goal.
    2. Check to be sure your goal is big enough to generate 3 weeks of activity. If not, adjust your SMART goal accordingly.
    Recording your activities and progress in a log.  The last page of the sample behavior change plan has an example of the log format you must use.  The example shows that your log MUST contain 4 columns in which you write the following essential info:

    1) the date of each entry

    2) details of each activity undertaken to achieve your SMART goal and objectives, such as date, time, duration, and the name of the activity.  Also include details about the nature of the activity, such as the intensity, mode, progression, your feelings, whether you were alone or with someone, etc. (anything that helps you evaluate how and why you’re progressing).

    3) the objective addressed by each activity

    4) evidence (signatures of witnesses, receipts, printouts from websites, photos, etc.)  If the evidence cannot be imbedded in the log, then the log should clearly indicate where the evidence can be found (page number, "photo A", "printout #1", etc.) with the evidence labeled accordingly.

    Failure to turn in a log that CLEARLY outlines these 4 pieces of info for 3 weeks will result in no credit.  Failure to complete this assignment will result in failure of the course.  It is expected that you will spend a considerable amount of time accurately logging your progress and activities.  Logs that reflect poor record keeping will be reduced in point value accordingly.

    1. See sample behavior change plan to be sure you complete all the components (9 total) of the Behavior Change Plan.  For full credit, all sheets must be stapled together, have the required cover page, and be placed in The Box.
