Assignments for the Week

  1. Take quiz in D2L (open Monday, Sept 29 @ 12:01 until Sunday, Oct 5 @ Midnight).  The quiz will cover Chapter 5 and concepts presented in class this week.
  2. Complete the Informed Consent form in class
  3. Make an appointment for a fitness assessment using the online system. Signing up for the appointment is due by Monday, October 13, 2008 at noon. Signing up by this due date will earn you 3,000 points (see page http://wellness.uwsp.edu/hpw102/grading.asp).  Completing the fitness test earns you another 3000 points. If you miss the due date for signing up, you still must make an appointment to get tested to pass the course, but you will only be able to earn 3,000 points for getting tested.  Notice on the table of assignments (http://wellness.uwsp.edu/hpw102/tasks.asp) that making an appointment and getting tested are 2 SEPARATE assignments with separate due dates.  The fitness assessment must be completed by Friday Dec 5, 2008.

During the first week, you completed a Fitness Medical Release Form that was evaluated by a team of physicians to give you clearance to have your fitness assessed. Signup for a fitness assessment is done online.

To sign up for your fitness assessment, go to this link to the appointment system and follow the directions. Be certain of the time you choose. You cannot change your appointment after you make it so be very sure you can make the chosen time before you confirm your choice.

Fitness assessments take place in room 033 Quant (Exercise Physiology Lab) in the HEC (Health Enhancement Center).

NOTE: If you miss your scheduled appointment for the fitness assessment, you must wait for make-up times to become available late in the semester. You will also be charged a late fee of $6.50. Be sure that you can make the time you schedule. Write down the date and time in your personal calendar so you don't forget. Do not schedule your assessment during class time!

    To Check Your Appointment Time
If you forget your assessment appointment day and time, just sign back into the system as if you were making a new appointment. It will tell you your appointment time.

  4. Begin reading Chapter 4 for next week.
