Your Health InformationThe University Health Service staff, the Human Performance Labratory staff, Dr. Wetter, Dr. Schoonaert, and their office staff all have access to your medical screening form ("Health Service Physical Activity Medical Clearance Form") during some time period. Here are the policies of the above entities regarding their use of your health information on the medical screening form. Confidentiality Statement from the University Health Service prepared by Dr. HettlerThe UWSP Health Service has a strict confidentiality policy regarding your medical records which essentially is that no one can have access to any of your medical records without your written permission. There are some exceptions to that policy (e.g., if there is a court ordered subpoena). The University Health Service uses this information to do medical screening. Since this information becomes a part of your medical record, it may later be used to carry out treatment. You have a right to request that the Health Service restrict how your health information is used or disclosed, but we are not required to agree to the request. However, if Health Service agrees to your request, we are bound by that agreement. Our fitness assessment is very confidential. The only people that have access to the information on the medical history forms are the Human Performance Laboratory staff, the senior level health promotion students who conduct the actual physical assessments and the health center staff who keep the forms as part of your medical records at UWSP. As instructors of the class, we want you to know how you are doing in the physical dimension of wellness. We do not see your assessment results as they are your private property. The assessment staff will let us know when you complete your assessment so that the appropriate points can be added to your grade. Your prior permission is needed to share the medical history forms with anyone not listed above. Signing the form gives permission for our office staff to record the fact that you have done the assignment and provides access to the people who need to evaluate your risk status prior to the physical assessment. |
