Physical Exam and Pap Exam Requirements

These must be done in the order they are listed:

  1. Health Issues Class – this is a review of important men’s and women’s health issues including your exam, review of your health risks, contraceptive methods, etc. You must view a 45-minute Health Issues video, which is a one-time requirement. You may complete an exemption form and be waived of this requirement by a Health Service staff member. The video may be viewed at http://wellness.uwsp.edu/. If you have taken the Healthy American class within the last year, this will also meet the educational requirement.
  2. Complete a health history or PE form including immunization record.
  3. Fill out name, date of birth, phone and address on ALL enclosed forms. Wherever you see file no. or ID no., put your complete social security number.
  4. Put your local address on the enclosed envelope and your PAP results will be sent to you (females only).
  5. Sign up for your lab work by stopping at the Health Service Front Desk. Women should not have their lab work done during their menstrual period.
  6. Turn in all of these forms to the lab personnel at your assigned time.
  7. Do lab tests (Urinalysis & Hematocrit). Hematocrit is a blood test that checks you for anemia (iron count). All lab results will be discussed at your appointment.
  8. Make your appointment by stopping at the Health Service Front Desk. If you are making an appointment for a PAP and pelvic examination, do not schedule it during your menstrual period. To assure a quality PAP test, it is recommended that there be nothing in the vagina 48 hours prior to your exam. Please avoid intercourse, douching, tampons, or vaginal creams.
  9. Read the enclosed STD information and decide if you need to be checked (test cost is $16.50 & no needles).
