Pap Instructions
Time is one of our most valuable assets. We will try to minimize your waiting time and still provide you with personalized, high-quality, professional health care. This may mean the use of self-completed history forms, care by a nurse or other practitioner, and listening to educations sessions with fellow students.
If you have questions about particular spaces on the form, go to our PAP form Question and Answer page.

It is highly recommended by Health Practitioners that students be aware of their cholesterol levels. If you have had your cholesterol checked within the last three years, supply a copy of your results. Cholesterol testing can be done at the Health Service for a fee of $7.50. Your practitioner may require this if you are going on oral contraceptives. If you have any questions, please call the Health Service at 346-4646 and we will be glad to help you.
